Program Enrollment Criteria

You must complete the application below and pass an eligiblity verification checklist to receive a referral to the Jefferson County Workforce office.

Enrollment Criteria

Applicants for the Summer Youth Employment Program (SYEP) must be 16-21 years old, low-income or receive public assistance and have one or more of the following barriers to employment or education:

  • Deficiency in basic literacy skills
  • High School Dropout
  • Homeless/Runaway
  • Foster Care
  • Pregnant or parenting youth
  • Ex-offender

  • Valid Driver's License or State ID
  • Social Security Card
  • Proof of Selective Service Registration (for males 18-years and older)
  • Proof of Income (last 3 pay stubs or bank statements)
  • Proof of School Status (Transcript)

By checking this box, you certify that you are the parent/legal guardian of the minor candidate whose name appears above, and hereby give your consent for the minor to participate in youth employment programs administered by the Jefferson County Workforce Investment Agency (JCWIA) and The Dannon Project (TDP), which include the Summer Youth Employment Program (SYEP), from the date on this consent form until such time the minor is no longer eligible to participate in youth employment programs or you revoke this consent in writing. You further certify that all of the information contained within the minor's application is correct and true and that by enrolling your child in any TDP programs, you hereby give permission to JCWIA, TDP, and its partner organizations to photograph/interview your child. It is your understanding that this photograph/interview or portions may be used by JCWIA or TDP and partner organizations to describe, promote, and publicize its programs. You agree to allow your child to participate in this project without financial remuneration, and you understand that this releases JCWIA and TDP and partner organizations from any future claims, as well as from any liability, arising from the use of said photograph/interview. You understand that by enrolling your child in programs offered by TDP, you consent to the release of your child's education records to JCWIA and TDP for the purpose of verifying your child's eligibility for these programs. You understand that education records include first name, last name, date of birth, address, enrollment status, grade level, and any additional fields. You further understand that TDP will use this information for no other purposes than to verify that your child is eligible for its programs based on age and residence in the Jefferson County area and will safeguard it against further disclosure.
